How to Refer to Child Development Service

If you have any concerns regarding your child’s development, you are encouraged to refer into our Child Development Service.

You play the most important role in your child’s life. We will work closely with you to plan services together.

We will contact you to learn more about your child and your family. We will discuss your concerns, questions and priorities and assist you to develop a plan to support your child’s development. This will include strategies and ideas you can do with your child in everyday activities to support their development. Before we first meet, it may be helpful to think about and write down your questions, concerns and priorities.

With your permission, we will work with other people in your child’s life who can help achieve your goals (e.g. schools, child care). We will also provide information and links to other services in your community.

Or you can talk with your GP or Child/ School Health Nurse who can also refer you into our service.

Once completed, please check your local Child Development Service location here and drop off a copy in person or via email.
